Over 18 years old: Fill out the top part only
 

Under 18 years old: Parent/Legal Guardian's information on the top and bottom. The child's name only goes where it says "In consideration of..."

Group Waiver Addendum For groups of children under 18 years old. List all the children on the form. One "Waiver" form (from above) must also be filled out by the parent/legal guardian that's signing for the children. All the information on the waiver form (top and bottom) will be the adults information.
